Apex Wireless reserves the exclusive right to suspend or terminate the subscribed line for late payment, payment failure, or default on behalf of Lyft.


LINE LIMITS

Users may have up to four Voice lines on their account.

For each Voice line, users may pair a Watch line if applicable.

ADDITIONAL LINES

Tablet lines are allowed and do not count toward the four-voice line limit. Users may add tablet lines to their accounts, similar to Watch lines.

UNACCEPTABLE BEHAVIOR POLICY

Any use of abusive language, threats, harassment, or the submission of repetitive tickets or calls is strictly prohibited. Such actions will result in the immediate termination of the Driver's eligibility and may be reported to Lyft, Inc., as necessary, for further action, including removal from their platform.

Upon termination, users will have a single opportunity to retain the phone number associated with the program, provided they pay all outstanding invoices in full. Failure to pay or actions requiring legal intervention and/or collections efforts.

Apex Wireless of Texas and Lyft, Inc. reserve all rights to take appropriate action to protect the integrity of their platforms and the safety of their communities. Any subsequent communications received, other than device shipment acknowledgments, may be used in accordance with applicable laws to support enforcement of program terms and ensure compliance.

This policy is enforced to maintain a professional, respectful, and secure environment for all parties involved.

OFFER TERMS

When purchasing a Device subject to a retail installment contract, the subscriber will be billed monthly on behalf of Corporate Subscribers in accordance with the retail installment contract for 36 months, and a monthly credit for the Device shall be applied to Subscriber’s bill as set forth below, subject to the following conditions: in the event the Subscriber either (a) disconnects such line, (b) upgrades to a different Device, (c) suspends such line without billing, (d) changes the plan to a non-eligible plan, or (e) chooses to pay in full the remaining balance due under the retail installment contract prior to completing the full 36-month retail installment contract, for any line that receives this offer, the monthly Device credit for the remaining months will not be applied to the balance due for such line. For example, if the full price of the Device is $500.00, the Customer or Corporate Subscriber will owe the monthly payment of $13.88 ($500/36) multiplied by the number of months remaining on the 36-month retail installment contract, and not receive the monthly Device credit. Provided Customer or Corporate Subscriber activates a minimum of 25 Corporate Subscriber smartphone lines during the Offer Period, and thereafter maintains such lines, this offer shall be extended for the remainder of the Initial Term of the Agreement or a maximum of 24 months, whichever is shorter. If the Customer or Corporate Subscriber does not meet the requirements, Verizon Wireless reserves the right to remove the monthly Device credit from the lines that have received the offer

PAYMENT

Invoices will be emailed 7 days before the payment run (autopay), and declined payments will receive a 21-day grace period before services are suspended. Once payment is received, it can take up to 24 hours for services to be reconnected. Additionally, a $25 reconnection fee will be applied to the following month’s invoice. After 31 days of non-payment, numbers will be canceled, and any applicable fees will be applied to the final invoice and sent for collection.

A 2.9% credit card transaction fee will be charged for card payments, and 0.75% for ACH transactions.

CONTRACT PLANS WITH VERIZON

Where applicable, the contract term is 36 months for a Verizon Device Payment Plan offer; credit terms apply. Full Retail and BYOD rate plans have no contract, and you may cancel at any time with no cancellation fees. An early termination fee/remaining device cost/lost promotion will apply to users who cancel within 36 months of receiving a device offer from Verizon.

​Unlocked Devices: If you are participating in a rate plan that offers a discounted device, that device will not be “unlocked” or “owned” by you until all payments on that line have been fulfilled. To bring your device to another carrier within 36 months, you must pay the difference in discounts previously received to unlock the device early.

PROGRAM TERMS

  • Verification: Your Association with Lyft must be verified through our portal before you can access and order.

  • Each Lyft End-User is allowed up to # lines.

  • A soft credit check will be performed for requests including a Verizon device offer.

  • Lyft is not responsible for the quality of carrier service or service interruptions.

FEES

  • A $5 administrative fee per month, per line, plus tax, will be charged.

  • A $.0.75 Platform Recovery Fee will be added to each line of service monthly.

  • A $30 per line Program Enrollment fee must be paid before shipment or activation of services.

  • $25 Restore Services Fee

  • $5 Late Fee plus 1.5% each day until 31 days when the line is subject to disconnection

  • $25 Payment Recovery Fee for payments made directly to Verizon. The user remains subject to Collections activities under this action until billing is notified of the payment.

 

SUPPORT

All billing questions will be directed to Apex Wireless; this includes, but is not limited to, changes to payment methods, billing inquiries, termination requests, orders for new or additional lines of service, or replacement hardware devices. Verizon Customer Support cannot assist you with account-related questions.

Abusive behavior towards the Apex Wireless support team will not be tolerated and may result in account cancellation.

 

USE OF PERSONAL INFORMATION

We use your personal information for various purposes, including providing you with services. We use your personal information for the following reasons:

•         Process your orders.

•         Respond to legal processes and emergencies.

•         Develop or inform you of new products and services.

 

INFORMATION WE SHARE

We may share information that is anonymized or in an aggregated form that does not directly identify you. We do not share your personal information with third parties:

Group Account Holders We offer group accounts to business and government customers, family customers, and other group account holders (“Group Accounts”). The account holder for Group Accounts is the entity or person that buys the service or product for its employees, family members, or other authorized users. You (as the user of a device) may receive service, certain pricing, terms, or other benefits through a Group Account with us. If so, we may share with that Group Account holder customer registration and other information related to your account.

PROTECTION OF APEX WIRELESS AND OTHERS

We may access, monitor, use, or disclose your personal information or communications for the following reasons:

  • Comply with the law or respond to lawful requests or legal processes;

  • Protect the rights or property of us, our agents, members, our customers, and others, including enforcing our agreements, policies, and terms of use, and responding to emergencies;

  • Initiate, render, bill, and collect for services; or facilitate or verify the appropriate calculation of taxes, fees, or other obligations due to a local, state, or federal government requirement; or determine eligibility for government benefits.

NETWORK AND INFORMATION SECURITY

We maintain a variety of physical, electronic, and procedural safeguards. These safeguards help protect your personal information from loss, misuse, unauthorized access, disclosure, alteration, and destruction.

UPDATING THIS POLICY

This policy may change from time to time, and the current policy will always be maintained within the Selfcare Portal, where you pay for your monthly service.

DRIVER ACCOUNT TERMINATION

I understand my Lyft reserves the right not to release my numbers in the event of fraud or any other unlawful acts under state and federal law. Lyft may designate me as no longer eligible for the Evergreen program services if my association with Lyft should be terminated.

Notes:  Current coverage details and additional plan information can be found at www.verizonwireless.com. Coverage includes the Verizon Wireless 5G Ultra Wideband, 5G Nationwide®, and 4G LTE networks. For the avoidance of doubt, the Verizon Wireless 5G Nationwide network is a separate network from the Verizon Wireless 5G Ultra Wideband network. Usage outside of the United States will be billed at the international travel feature rate, if applicable, or pay-as-you-go pricing, which can be found at www.verizonwireless.com/international. Lines activating on this plan must be on 5G Nationwide or 4G LTE smartphones.

(1) After 22 GB of data usage on a line during any billing cycle, usage may be prioritized behind other customers in the event of 4G and 5G Nationwide network congestion. A 5G Device is required to receive 5G Ultra Wideband (UWB) service. To ensure users can maximize their high-speed data use for business applications, video applications will stream at up to 480p, and will apply video streaming up to 4K when in 5G UWB network service areas.

(2) Mobile Hotspot is available on all capable Devices and allows the line to share data allowance with multiple Wi-Fi-enabled Devices. If 10 GB of Mobile Hotspot data usage is exceeded on any line in any given billing cycle, Verizon Wireless will limit the data throughput speeds to up to 200 kbps for additional usage while in 4G and 5G Nationwide network service areas for the remainder of the then-current billing cycle for the line that exceeds the data usage.

(3) Includes domestic toll-free calling and toll-free calling from the United States to Canada and Mexico.

(4) Unlimited Messaging from within the United States to anywhere in the world where messaging services are available. For other messaging rates, go to www.verizonwireless.com. Verizon Wireless will terminate a line if more than half of the usage over three consecutive billing cycles occurs outside the United States, after 30 days’ notice to the Customer. 

A brief service outage and Device reboot may be required for this plan to take effect on existing 5G UWB lines.

**Lines on this plan must be activated on a Device provided by the Customer or Corporate Subscriber, or purchased from Verizon Wireless at full retail price, through a Device retail installment contract, or via a discounted Device offer with a Line Term.
